[MVMA] nc8q-raspbx: Dial-by-callsign feature

nc8q-raspbx (FreePBX, Asterisk) conforms closely to 'Meshphone' by Mark 
Herson N2MH.

Apparently due to the loss of W8BI/DARA nodes, we have lost the tunnel 
to the N2MH Meshphone service.
IOW, the 'long distance' feature is not available.
Also, Clinton County WX8A/W8GO nodes are not available.
I have backup tunnels to WX8A and W8GO enabled and as soon as their end 
enables the tunnel,
AREDN (and telephone) service to Clinton County will be restored.

Dial-by-callsign feature

If you don't remember what extension a registered phone at nc8q-raspbx 
has or have a telephone directory
(Whitepages.htm) handy, registered phones may use the dial-by-callsign 
feature.
Dialing feature code '73' will bring up a new dial-tone and then
the last 4 characters of a callsign may be dialed.
In many cases the full callsign may be dialed.
Many unregistered phones may be dialed with this feature.
