[MVMA] Hamshack Hotline <> Meshphone

Mark, n2mh, has a Hamshack Hotline <> Meshphone switch.

Folks with a Meshphone registration (locally either nc8q-raspbx or w8bi-pbx)
can dial a Hamshack Hotline number and
folks with a Hamshack Hotline phone can dial a Meshphone number.

e.g.
 From Meshphone dial 78# 577-4847 to reach my Hamshack Hotline phone.
 From your Hamshack Hotline phone dial 4508, 78, 9371073 to reach my 
Meshphone.
 From your Hamshack Hotline phone dial 4508, 78, 9371[01]xxx to reach a 
local Meshphone.
