[MRCA] AAF radio ops logsheet

I would like to get ahold of one of the aircraft radio operators log sheets (AAF form 35) 

I would like to finish off the working display area of the shack with this.

Have a little set up with a SCR-522,ARC-3,BC-375 and ART-13 and other sets as time/parts permit.

I have looked in the ROIF and other sources and they are nut too legible.Thought the web would have a PDF...or something but to no avail.

Static has settled down quite a bit in SW,Fl so will get on the air more often now atleast with one of the HF sets.
