Thanks indeed, Lee. That settles that. 
I have a bunch of radiogram forms, even some new pads full. I think ( eventually ) I'll put an example of each one in a binder in sheet protectors. The one that seems most uncommon is a form
from the "Red Salmon Company" in Alaska. 
In the not-recent past I promised to give out some examples of these forms. I still have them and this seems a decent idea, as several people expressed interest and I did promise them. 
However I'm less inclined now to take on more work and I think when and if I do this, I'll have it on the basis that they supply me with a mailing envelope ready to go. 

Re the KPH and K6KPH that was mentioned. I have a Mackay 401 lifeboat radio; this is a tubed set with about 10 watts input, and has A1 also on the 8 MHz band with a tunable receiver. I think
it would be an extreme kick to contact K6KPH using that thing, recrystaled for 40 meters and with the receiver tuning padded down to 40. But that's another project behind many priority tasks.

I can help with this. First of all, some cargo vessels did carry pax; in some cases, the wives/children of the ship's officers, while paying passengers were carried on some other vessels. Secondly, the captain and crew also would send radiograms for everything from requisitioning to Health and Welfare; these would all be on forms and the originals would be filed in the ROs files after sending - or after sending a reply. It was critical that this be done so that the bean counters could check the charges against the ledger from RCA or ITT Mackay (you either used the RCA or Mackay stations).  The only times that a ship station wasn't charged for passing traffic to shore stations was AMVERS, OBS, and communications preceded with a TTT, XXX, DDD, or SOS (DDD was sent ahead of SOS when a station was relaying or replying to a distress call). 

To answer your question, it was likely an unused form from the ship itself........
