I think I'd want to double check that statistic on dud rate. Can you imagine the massive bomb disposal effort this would have required, if it was true?
The only conditions I have read of, where there was a high fail rate, was were the soil was mud. Even then I don't think fail rate approached anywhere
near 16%.
