If Torpedo caused alarm, note maneuver of ship and
how  far ship turned relative to original course before
Torpedo passed bow and was Torpedo seen, as listed
in A, B, C, D, E above.

Note other pertinent data such as: length of time alarm
sounded, your estimate of distance Torpedo was fired,
etc., if possible, also remarks.

End quoted material. What is this about? Just a written
report form, but why the question "was Torpedo seen", if it
was a sighting report? Some kind of automatic  torpedo
