[Milsurplus] Question about AN/ARC-38 Code Book.

I have a code setting book for the ARC-38.  Spiral bound at the top of the 
pages.  About 4" x 5" pages.  Last page number (here) is 352.  This copy has no 
covers.  First page (not numbered) has instructions.  Second page (not 
numbered) has Figure 1.  If anyone has a book with a front cover, I'd like a copy or 
at least to know what is printed on it including pub number and date.  Also 
whether there are any pages missing from this one.
