[Milsurplus] OT: Any VoIP Gurus Out There?

I need some advice from someone who knows about setting up
a dedicated server for VoIP, similar to Echolink but with
more bandwidth up and down per user (only going to have 2-3 users
at a time, anyhow).
I'm no "bit head" so I'm swimming in murky water here.
All the "rent me cheap"
voice servers have way too much latence- over a minute
with just a 22k stream, right off..ewww.
The Yahoo chat server does exactly what I need and it works,
but I can only get about 3 KC of bandwidth out of it.
I bought the new ARRL book, but it was really not
much help.  I don't want to set up Echolink;
I need VoIP for a specific application.  20K of
through-put both ways will work; 48K would be better.
People stream video conferences in real-time,
so *somebody* knows how to do this.
