That plug was in production for a very short time in the US in the 60s, IIRC.  It's design is now not permitted by the National Electrical Code, so you are going to have to get someone who has a piece of equipment with one installed to look at the manufacturer's name on it.  I thought I had an example around here, but I cannot find it.  Where you go from there, however, I cannot suggest.
