[MCARC] ceramic wafer switch

I'm looking for what I believe is called a 5x2 wafer switch. On the same side of the wafer it has 5 positions plus a common connection for an in or out feed. It has two of these on the same side and they are not electrically connected. Kind of like the pix only this one is a 3x2 and I need connections all on the same side. It is need for a tuned input for the GLA1000 conversion from 4 sweep tubes to a Russian ceramic triode. I have started this project back before 2010 and I'm back on it again. Thanks for looking.
