[LOGic] Lost my CQZ entry field

I made a  boo-boo.  I unlocked my log form to put in a field to 
record cards received from SWL's.  I don't know what I did but 
suddenly my CQZ field disappeared, both the field itself and it's 
label.   I remember that this had previously been set up as the user1 
field but it is no ,longer listed on the form to set up user fields.

Did I only lose the field on the form or did I also lose all the data 
previously entered in that field for hundred of entries?

How do I restore the field on the form?

If I lost the data as well what, if anything can be done to 
restore?  (I don't remember when I did my last backup.)
