The secret is to be able to confirm what you're looking for using
Filter in your logform.  Go to the Filter form and click on Edit.  Try
an expression like:

TAG_VAL("YL MEMBER:", USERFIELDS, "C")='Y'

If this works (meaning you get all the YL MEMBER records) then go to
Print|Log and then enter in YL NUMBER: in the optional fields to
print.  Then enter in the expression from above in the box right
before printing.
