[Lafayette] HA-800?

The matching included speaker is worth more then the current big price.
This thing even includes the original manual. Owner claims low
sensitivity. Probably shorted screen bypass or coupling cap between
stages. Unless it took a lightening hit and burned out all the front-end
coils. Looks decent.
