Good Morning,
The net control in Western North Carolina appears to be using an ARRL Radiogram form or at least the concept of this form to pass Health an Welfare check messages. It is a standard method, that everyone follows to very efficiently exchange information very quickly. If you listen to the exchange of information. You will find that it is not unlike a lot of the exchanges in contests like field day or QSO parties. It is also similar to the exchanges made during DX contacts. A fill-able PDF file version of the ARRL Radiogram can be found here:
https://www.arrl.org/files/media/Group/Fillable%20Radiogram%20Form.pdf
Listen along to traffic and see if you can fill out this form. Listen to the LIVE audio feed at:

Basically, amateur radio operators, or in some cases non-amateurs using ham radio in an emergency, are calling to net control with welfare checks. The control takes the following information.
Name of person to be checked on.
Their last known address or location.
Name of person requesting the welfare check.
Their contact information.
The call sign of the person relaying the information.
He writes this information down and then asks for an amateur in that area to go check on the person. Relaying all of the same information to the mobile unit. The mobile unit attempts to locate the person and relays information back to net control in the same format. Net control then relay's the information back to the original source again using the same format of information.
Listen to how fast these exchanges occur and how net control pass the information along to mobiles in the field. Can you do it?
Brian
