How about instead of a PDF form you just make a form in WP that enters the information into a database or emails the information to the secretary?

A new JOIN CARA page has been Added to the website... On this page there are links to NEW MEMBER APPLICATION and MEMBER RENEWAL FORMS.  This page is in the sub menu under ABOUT CARA menu selection.

I will be working to improve the PDF forms so they can be filled out electronically and e-mailed in without having to do a lot of manipulation.  (It may take a while, though.)

If you have any suggestions for improvements, please let me know.  73,
