[ILQSO] How to specify my e-mail address?

Please straighten this LID out. I'm reading the rules for ILQP, and it 
talks about having a summary sheet in addition to a cabrillo file.

Is this correct?  Where do I find the summary sheet?  I don't mind 
filing one out, but in the rules there's no reference except for folks 
who send in paper logs.

And, if I only send it a cabrillo file, how do I let folks know my 
e-mail address within the file?  I looked at cabrillo specs on arrl.org 
and it doesn't say anything about email address.

Of course, I didn't look too deeply into cabrillo, because I assume 
logging software like the incomparable, gold standard, N1MM+ super 
software would know about these things.

Would someone from the ILQP organizing team straighten me out?
