For "frequency", I've had questions about what is required.  Basically, anything that identifies the band and mode you were using.  "40", "40M", "7000", "7238" are all ok for "frequency".

For "club", if you operated within IL and belong to an IL local club, please include this as your "Club Affiliation",  not Society of Midwest Contesters (not a "local club").  SMC is not eligible to win the traveling plaque.
