There are 4 pins on the CR-64, two are for the resistor heater and two are the crystal. Generally you will have to remove the existing Crystal from the PLL unit, I think it is a 30.72 MHz. clean out the 4 holes, orient the CR-64 so the Crystal is installed into the correct holes, solder it in place. Let it warm up for 30 minutes or so and then adjust it onto frequency.I do not recall any extra jumpers being required.
The CR-64 was used in so many Icoms it is doubtful an instruction sheet was produced for the 275, 475, 575 series. The CR-64 was also used in the 751, 751A, M700 and M700TY transceivers and possibly more it has been 15 years since I worked there and supported Icom products.

Good day list members,
I recently acquired a used CR-64 pulled from a working unit being parted
out, and want to install it in my IC-275A.
The manual has nothing more than a 'it goes here' diagram, and no
information as to orientation, possible jumpers (if needed), etc.
I've done the online search and not turned up a copy of the installation
sheet.
Wondering if anyone has a copy of the installation sheet somewhere in
their archives, and could scan/photograph the sheet and send via email?
If so, it would be very much appreciated.
