These are standard 1/4" mono-audio Y-adapters. They are available almost
anywhere audio products are sold.
Since you are only using the tip and sleeve, you could also use the stereo
Y-Adapter versions too as the ring would be shorted to the sleeve.

Got one a Radio Scrap... but it was last year.
If it's a mono 1/4" jack you could also try a music store.
