Dan, how many times has the headset jack been used?  Possibly bad connection 
within the jack itself.  Seen that happen on several radios, which 
investigation found the jack ... really was a cheapie..!  Tended to lose 
tension after many repeated insertions of the headphones.

One thought anyway..
