[Icom] Phone Patch and the IC 706MKII

Has anyone ever hooked up a phone patch using a 706MKII?  I really want
to get one up on the air and cannot figure it out.  
 
I first tried an MFJ patch, but it did not work.  Then I bought a
Kenwood phone patch and changed the mic pinouts to match (or so I
though) the 706MKII, but no dice.  Since I bought that patch off ebay, I
am not sure it works anyway.
 
I would appreciate any advice I can get.
