You will likely have to make your own. The Acryllic forms become very 
brittle with age. I have a GDM and the coil forms are becoming very fragile. 
You can likely turn up the 3/4" tube from Small Parts, the trick will be 
finding suitable pins for them and finding a way to mount them. Since its a 
HB meter, maybe you can find an Octal tube socket to replace the one there 
already, then break up some junk octal tubes for the plugs.
