Thank you Ron, you are absolutely correct.

When I downloaded directly to Adobe Reader all pages were properly included.

For some reason my Firefox download got a blank page instead of the page 
labeled 5.  Tried more than once, same result.  For that, it may be time 
for me to change browsers.
