[Heathkit] Heathkit Tube Tester Data Sheet

i purchased some Heathkit equipment today, among them a TC-2 tube
tester;  inside the manual was the "Supplement Test Data Sheet", dated
1-1-64, which is the 14 page booklet that is fairly common to the
listers here; also included was a single sheet "Test Data Sheet for New
Tubes, for use with Heathkit TC-1, TC-2, TC-2P, TC-3 & IT-21 Tube Check-
ers", which i haven't seen before; it is dated 4-1-64; also at the top
of the page is "597-154".
i have scanned the page, anyone wishing to receive the scan (.gif for-
mat) feel free to email; the original was folded/creased/torn but is
fairly readable.
