After reading your email, I gave Jay a call.  He's been in and out of  the 
hospital and during that time his computer crashed.  Unfortunately,  he's 
just not up to dealing with fixing his computer at this time.
 
However, here is what he suggests on the book:  Go to his web site, 
_www.w6hht.com_ (http://www.w6hht.com) , print out the book order form  complete it 
and send it with payment ($10 domestic, $15 international if I  recall 
correctly) via snail mail to his home address:

If anyone else on the reflector is still waiting for the book, Jay says  
he's well enough now to fulfill the backlog of orders he has accumulated since 
 his surgery.

working.  But, I haven't heard back about whether the CD is still available
from  him.
