Joe: The way you stated your speaker/headphone problem sounds confusing and backward... The only time any B+ should appear in conjunction with a speaker would be to switch in an electromagnetic coil generally used as a choke in pre-war sets. Never to the voice coil.Switching IN a transformer would be  helpful to match plate to voice coil impedance. I have seen B+ appear across a pair of high impedance headphones in a output plate circuit.
One of my later model super-pro sets requires the use of a transformer to match voice coil impedance (low), but not to match headphones of say, 2000 ohms or more. I do not have your schematic, nor mine in front of me so, I can't comment further on the particulars of your situation.
