Glen is generally correct here.   There is no need to provide a 
copyright notice on documents, since most of the provisions of the 
Berne Convention were adopted by the U.S. when the Copyright Act was 
amended in 1976.  Also, the term is extended to 100 years under the 
"Sonny Bono" Copyright Extension Act of 1996.

Generally speaking, a work is copyrighted when it is "fixed in a 
tangible medium of expression".  You see copyright notices on documents 
because they may be circulated in countries that do not adhere to the 
Berne Convention provisions.

For works created before 1976, (and hence, under the 1902 Act) the 
analysis is a little more more difficult, since it involves discrete 
terms that require renewal in order to maintain the copyright as 
enforceable.

With Trademarks, entirely different laws apply.
