       whether or not the editor is opened

     * corrected hamcall.net account access

     * changes to logins report format
       . allow user to select which fields are included in the report
       . add header row
       . allow user to select the column order in logins report
       . output is tab delimited for importing into a spreadsheet program
       . change tab delimited output log file extension to .csv
