Fonts are held open by the system and other software most of the time. A system crash, application failure or similar can occasionally corrupt fonts on disk so that they start acting up. Microsoft Word used to often be the culprit as it failed regularly requiring a force-quit and restart.
Also, as system and software updates come through errors in older fonts themselves may show up which were ignored by older versions of the system.
Another problem that often arises is applications that install their own versions of already installed fonts causing duplicates and conflicts.
The nasty thing about font problems is that they can cause weird errors in different applications that can be hard to track back to the font in error. Sometimes a system can be acting slightly weird for months before the problem is found.
