Hi David and all:

Just a quick tip if you open up the Console application located 
/Applications/Utilities it should report some explanation as to why it 
not running.
If you have the console application open you can see it update live 
from the system.log and console.log files.  This should help narrow 
down the issue. Could be something simple like application doesn't have 
executable bit set or permission are set incorrectly.
Great tool for correcting problem with permissions is Batchmod 
(Freeware).  You can find it at www.versiontracker.com.

If it something more complex you will at least have some explanation as 
to why it not running.
This works for both 10.2 and 10.3 system but is nicer and easier to 
navigate logs in the updated 10.3 version.
