Fairly common to use xxx.jpg.exe (or similar) too - if there are two
"extensions" Windows only sees the last one. If the user has the system
configured to "hide known extensions" then it appears as "xxx.jpg" (a
picture) - but won't display the picture. However, if the "real" extension
is .wsh (Windows scripting) or similar, then the script can be used to
"show" a picture, then run something else as well.
That's the simple way - only fools those who don't know to look for it, but
someone who knows how to write real programs can do the same thing but hide
it a lot more effectively. HTML emails with embedded script and attached
executables can be very sophisticated.

Executable images have been around since the 80's for all PC and
compatables, used a lot when image viewers weren't easy to come by. Show the
image, run the .exe, .bat, .bas et cetera and vice-versa. I have a program
for it around here somewhere... A nasty little trick.
