A couple of times I ran into this, it was a registry permissions problem - just like many file systems (e.g. NTFS), the Windows registry also has permissions security. Generally, the "hive" or registry "branch" in question is set to "read only" and the installer expects to have Full Control.
InstallShield is published by Macrovision (yes, the same Macrovision that develops DRM (copy prevention) products). Check these two KB articles if you haven't done so already:

The second link includes links to re-install the InstallShield engine, but you need to determine what version of InstallShield is currently on your system.
Another thought is a corrupted registry or possibly logical drive or file system errors (which might mimic a corrupted registry issue). From a Command Prompt, type "CHKDSK x: /F" (without the quotes) where "x" is your boot drive (usually "C"). CHKDSK will balk because the boot drive is locked - answer "Y" to run CHKDSK after a restart. Then close the Command Prompt and restart the system - CHKDSK should run before the desktop GUI loads. Don't press any keys before CHKDSK starts or it might abort the check. Once CHKDSK finishes (it may take a while) and you're back in Windows, try installing your app again - if it still errors, then read through the two KB articles.

The OS is Windows XP.
Suddenly I am unable to install from a .exe or uninstall programs. I get an
InstallShield error 1607 which has to do with a missing or corrupt
ISShield.msi file. There is no trace of it on the computer. Following
various instructions on microsoft and other websites does not help, so far
no information is accurate, files names, locations, et cetera.
How can I get InstallShield to work again? Where can I get it from and just
re-install this program? My wife spoke to a co-worker, one of the computer
geniuses for the state and he said he has heard of the problem before and
the best bet is to reformat and restore the HD, why can't InstallShield just
be put back on and use as normal?
