I've also experienced most of those random problems over the years.  I did
track down that nearly all my mouse related problems were caused by using in
optical mouse near flourescent lights!  I found that placing my hand over
the mouse and blocking the light stopped the problems, even just a shadow
made a dramatic difference.  I switched my desk light to an incandescent,
problem solved.  If I should ever be in an area with flourescents again, I
will pain the inside of the mouse case black.
