I don't use OL Express, but two "general" things come to mind...
1. Make sure the MS Office dictionary files are properly installed.
2. If you have a custom dictionary where you added additional words, try renaming (or deleting) it. Corrupt custom dictionaries are known to cause some problems with Office products. The default filename you're looking for is CUSTOM.DIC. Location varies depending on OS version, so just do a global drive search for it starting a "C:\". It will be recreated automatically if needed, but all previous custom entries will be lost.
If all else fails, do a search on Microsoft's support site at http://support.microsoft.com. Use the keywords "Outlook Express spell check". This should get you started.
