Looks like a straightforward single plate to P-P grids unit to me.   
Antique Electronics Supply in Phoenix <http://www.tubesandmore.com/>  
has what should be a suitable replacement.

If the problem with your original unit is an OPEN primary you might  
try the following quick-and-dirty fix.  Connect a 100K resistor across  
the primary and run a .05 mfd cap from the plate end of the primary  
over to either of the grid leads on the secondary side.  Warning, this  
won't work if your secondary is open or if there is a primary-to- 
ground or a primary-to-secondary short.
