I have what must be that file, and will next forward it to Clay (and  
others that might want it.)

My guess is that it got removed during a storage down-sizing effort.

The PDF file has been OCR'd, has perfect clarity in text and  
illustrations, and the schematic is very readable, though needs to  
have it's component values added.
