FWIW, the chart in my Allied Electronics Data Handbook (1961), based on 
Mil-C-91A,  doesn't completely agree with the chart Craig referenced.
For the tolerance band this handbook shows Silver, not white as +10%, no 
option for 40% tolerance, and only one stripe used for voltage rating.
