Have you ever seen a photograph of an electrocution chair (with or without 
the convicted person)?  They had a helmet like thing that apparently 
assured lots of current from the head to the rest of the body.

Same with the S-38.

The headphones jack is connected to the output transformer secondary. The 
output transformer primary is connected to the B+, which is connected via 
the rectifier tube to the power line hot side.  The primary and secondary 
are separated by a few layers of 60 year old paper insulation.

Think about that.
