A close look seems to indication that the punch pins are all going a little too far.  The hinge side of the chad appears to have some tearing that raises the hinged side above the rest of the tape.  Worse on #5 pin to the point of tearing the edge off.

I see a screw labeled upstop screw.  That looks like it would be the adjustment to change the stroke length of the punch pins.  I haven't found the paragraph which describes its adjustment yet.  (figure 49 in 165B PDF page #82)  Figure 44 on PDF page # 83 appears to indicate the allowed difference in distance between the bottom of the punch pin and the punch selector fingers.

I think I need to do a little more research because I think I see another possible place to make an adjustment.

I seem to be closing in on this one, but the #5 punch is shall we say a little aggressive.  This is a chadless punch, but I'm getting chads at times from #5 punch pin and it ends up at times having that edge of the tape splitting off.  Any adjustments I've identified would seem to affect all 5 pins.

Oh I almost forgot, perhaps the old semi brittle tape contributes to the problem?
