[GreenKeys] Man readable tape leader

I'm pretty sure it was in the ARRL's "Specialized Communication Systems (for the Radio Amateur)", one of my treasured reference books when I was a teen ham.  If nobody else comes up with it by then, I'll try to scan it and put it up later tonight.
