 I use voip.ms as a voice over IP service provider. Not free but very affordable.Look at some of the Asterisk groups for ideas on connecting hardware.ATAs (Analog Telephone Adaptors) can be found cheap.
I think there may be special considerations for getting a fax or modem to work over VOIP.I have never done it but I think it's possible.
