Go over to the HSH help desk and create an account.

The reason for registering FIRST is that a guest can't upload a picture.

Once you have completed the registration, Open a new ticket.  The help 
topic will be New line request for the area you are in - Presumably HHUS.
When you create the ticket, fill in all the requested information. You 
will have the option to upload the photo.  It will take up to a few days 
to be issued the number.  When they do, they will send instructions on 
how to configure the phone.
