Back when those cords and panels were used, 
the operators did not rely on everything being 
done for them ... they learned (maybe the hard 
way) what to do and not to do. 

Like with the recessed brass screw ...  you grab 
the long plug handle on the back end so your 
thumb is nowhere near the little screw ...  

As for loops being floated or grounded, I think 
most LOCAL (in the 'shack') loops were floating, 
but I know that WU often used earth ground 
return for their longer outside loops. [Saved a 
bunch of $$ on wire !!]
