I won't dispute the findings on this website.  Toward the end, though, they infer that

      ". . . high processing temperatures"  are necessary for this reaction to occur.  

If a fault arc were to take place inside a transformer with 13,200 volts on the primary, the
temperature might be high enough for some phosgene to be created.  THAT was what the rep from
our transformer maintenance company was talking about.  The transformer leaked some PCB oil
on the floor, and when this was noted, the transformer was taken out of service by closing a
tie breaker to the alternate feed side.  The 13,200 was removed and the area isolated to 
prevent any unnecessary foot traffic.

That was when 'silly Ralph' decided to open up that transformer for 'a look inside'.  I don't
know how much phosgene is required for one to be 'in trouble', and I'm not interested enough to
experiment.  Had I known about the remote possibility of  PHOSGENE  being inside, I would have
taken my tools out of the area and found a different place to wait between assignments.  

It was the query of a maintenance cleaner that got everyone's attention.  He asked if it was
OK to clean and mop the Primary Room, including the 'stuff on the floor' near the transformer.
It didn't take long to see the source of the 'stuff on the floor'!

The fact that no noise was heard prior to the discovery of the leak and no fluctuations of
voltage were noted by the equipment fed by this transformer, the leak was probably related to
 a leaking fitting, recently.  It was 20-some years between installation and leak discovery.

Other than red tape and a number of 'meetings', our boss was not bothered much by it.  His
background was in steam boilers, not power generation or distribution.  At our location there
was no 'power generation' other than by the rented generator.  Our Power House distributed
13,200 volts all over the manufacturing plant area, that was transformed down to 480V for
connection to machinery on the plant floor.  We had  25 remote substations with one to
three 2000KVA transformers each and associated 480V switchgear.

I do appreciate you seeking out the reference below.  Until I read it, all I knew was what might
be considered second or third hand knowledge or wisdom.  (Read, rumor!)  Now it has some proof.
