I can confirm that -- have an NOS cover on an NOS FRXD here. 

It looks something like a 14 reperf cover but defiinitely is for the FRXD.

A Youtube clip called "Teletype FRXD" shows one with a cover (not mine).

 Housings for FRXD are quite rare, since they were mostly used in switching
systems where they were mounted in cabinets with other stuff.  But a cover
does exist - Jack showed me a picture of one after I had doubted their
existence.
