I made some. I use to have access to a paper guillotine (need again,
willing to buy one).  The spec is for 99lb paper stock, I used  "100 lb
cover, Sheffield 125", note that weigh does not always translate in to
thickness. A good supplier will know, it has to do with the density of the
stock. A card is 7-3/8" x 3-1/4" x .007". I forget the details of the notch.

I made the cards in the mid 1990's.

Any 'real' print shop will have a guillotine, depending on their stock you
may have buy a 36" x 36" ream.

If your lucky he will have smaller stock, 8-1/2 x 11 to 11 x 17.

You will want to have a few samples made first unless you know the paper
stock is for punch cards, the two most important spec are thickness and
smoothness. The wrong
smoothness and the cards will stick together.
