The "AP Style Book for Teletypesetter Circuits," which is online at:

has one example of a header for a TTS (not TTY) story, explained.
There must have been a similar AP book for 5-level TTY, but I'm unaware
of it.  They did use "eyeball" sequences intended to be readable
on the tape (for the story numbers).  Sam Hallas has a good writeup
