Yes.  It says TT-30/AGA-1 as I remember.  The motor is for 115 volts,  
but I am not sure if it is a governed motor or a synch motor. I'm  
quite sure there is no 25L6 tube in the thing.

Yes

No, I don't think so.  It's been a while since I've seen it (I moved  
twice in 3 years and am  now cleaning out the needed storage areas.)
