It's easy on the Mac with a free .pdf utility called appropriately  
"Combine PDF's":

Although I haven't used it on my PC (why do that if you can do it on  
the Mac?) here's a free utility that might work:

My Epson scanner will scan each page separately....once I've done  
that I simply use the combiner utility to meld them into one .pdf  
document.  It won't make the smallest collated files possible but  
hey, it's free!
