I was not there, but some folks from the then-National Bureau of  
Standards most likely were.  Do a Google search on "national standard  
ASCII" and you can read a lot.

NBS certainly published the standard as a FIPS - Federal Information  
Processing Standard:

This one is dated Nov 1968.  I have a copy of ASCII, and what I have  
is likely the ANSI version giving all the details.  The above FIPS  
officially adopts the ANSI standard and does not have tables of codes  
and letters and the like.
